Sleep Apnea: A Common Disorder



Sleep apnea is a common disorder that is characterized by very
shallow breathing while sleeping. The most common type of sleep
apnea is obstructive sleep apnea.



When sleep is upset throughout the night the next day can be one
of fatigue and sleepiness. A person with sleep apnea does not
get restful sleep because:

•Brief episodes of increased airway resistance and breathing
pauses occur many times throughout the night
•You may experience many brief drops in the oxygen levels in
your blood
•Poor sleep quality occurs because you move out of deep sleep
and into light sleep several times during the night.

Loud snoring is often a characteristic common to people who have
sleep apnea. Not everyone who snores has sleep apnea and there
are people with sleep apnea that do not know they snore.

Sleep apnea happen more often in people who are overweight,
however thin people can also have the condition. The majority of
people with sleep apnea do not know they have it, and they do
not know they are having problems breathing while they are
asleep. A family member and/or bed partner may notice the signs
of sleep apnea first.

An increase in the chance of having blood pressure which can
lead to heart attack or stroke is one of the consequences of
untreated sleep apnea. Sleep apnea left untreated can also
increase the risk of diabetes and the risk for work-related
accidents and driving accidents.

There is an estimation of over 12 million American who have
obstructive sleep apnea. More than half of these people are
overweight and most of them “snore” heavily.  Sleep apnea is
more common in men, and is more common in African Americans,
Hispanics, and Pacific Islanders than in Caucasians. It tends to
run in families as well. It can also occur in children. If your
child snores, you should discuss it with your doctor or health
care provider.

Common signs and symptoms of sleep apnea include:

•Loud snoring
•Choking or gasping during sleep
•Fighting sleepiness during the day (even at work or while
driving)
•Morning headaches
•Memory or learning problems
•Feeling irritable
•Not being able to concentrate on your work
•Mood swings or personality changes
•Feelings of depression
•Dry throat upon awakening
•Frequent urination at night

Treatment of sleep apnea:

The goal of treatment is to restore regular nighttime breathing
and relieve symptoms such as very loud snoring and daytime
sleepiness. Appropriate treatment will also help the associated
problems of high blood pressure and reduce risk for heart attack
and stroke.

Tips of self-help for sleep apnea include:

•Avoid alcohol, smoking and or medicines that can make you
sleepy.
•Lose weight if you are overweight
•Try sleeping on your side instead of your back.

One of the most common treatments for sleep apnea is continuous
positive airway pressure (CPAP). This treatment is wearing a
mask over your nose during sleep.

If you snore, treating sleep apnea may help stop snoring,
however, this does not mean you no longer have sleep apnea or
that you should stop using CPAP. If CPAP is stopped or not used
correctly, sleep apnea will return.
Possible side effects of CPAP include:

•Dry or stuffy nose
•Irritation of the skin on your face
•Bloating of your stomach
•Sore eyes
•Headaches

For people who have side effects working with sleep medicine
specialists and technicians can help reduce the side effects.
Some things that can be done include:

•Use a nasal spray to relieve a dry, stuffy, or runny nose
•Adjust the CPAP settings
•Adjust the size/fit of the mask
•Add moisture to the air as it flows through the mask
•Use a CPAP machine that can automatically adjust the amount of
air pressure to the level that is required to keep the airway
open
•Use a CPAP machine that will start with low air pressure and
slowly increase the air pressure as you fall asleep.

People with severe sleep apnea symptoms usually feel better once
they begin treatment with CPAP. It is important to follow-up
with your doctor and to discuss any side effects with your
doctor.

Other sleep apnea treatments include:

•Mouthpiece (oral appliance) for people with mild sleep apnea
•Surgery: The type of surgery depends upon the cause of the
sleep apnea.

Types of surgery include:

•Surgery to remove tonsils and adenoids if they block the
airway, this is especially helpful for children
•Uvulopalatopharyngoplasty
•Laser-assisted uvulopalatoplasty
•Tracheostomy
•Rebuilding the lower jaw
•Surgery on the nose
•Surgery to treat obesity.

Hay Fever Symptoms And Treatment



Hay fever or allergic rhinitis is a common allergic response to
specific substances in your environment. Hay fever is one of the
most common allergic conditions in the United States.

Seasonal hay fever is triggered by tree pollen, grasses or weeds
and other plants. Year-round symptoms can occur if you have
indoor allergy to dust mites, cockroaches, mold or pet dander.

Mild hay fever symptoms can be managed effectively with
over-the-counter medications. Year-round symptoms or severe
seasonal hay fever symptoms may require an allergy specialist
for appropriate evaluation and treatment. My year-round allergy
symptoms are managed effectively by an internist. An internist
is a physician who specializes in adult diseases.

Without proper treatment of hay fever symptoms there can be an
increase of more allergic conditions such as asthma or eczema.
Uncontrolled hay fever symptoms will impair the quality of your
everyday life.

Hay fever symptoms include:

• Runny nose
• Watery nose
• Congestion
• Frequent sneezing
• Itchy eyes, nose, roof of mouth or throat
• Swollen, blue-colored skin under eyes
• Postnasal drip
• Cough
• Sleeplessness
• Fatigue
• Irritability
• Facial pressure and pain

Hay fever symptoms are similar to the symptoms of a common cold,
however, there are differences. Hay fever does not mean you are
allergic to hay and is not triggered by hay. It does not cause a
fever. The probable cause is an allergic reaction to pollen or
mold. Heredity plays a role in determining who becomes allergic
including hay fever. If allergies or asthma runs in your family,
you may be more likely to develop hay fever.

You are most likely to develop hay fever during childhood or
early adulthood, but it can begin at any age. As you get older,
symptoms may worsen or improve. I have noticed my hay fever
seems to be diminishing; however, this could also be due to
better allergy medication.

Treatment for hay fever symptoms:

Your doctor will identify what allergy triggers your symptoms
and then develop a plan to avoid and/or cope with those
substances. Review the following treatment options:

• Nasal corticosteroids sprays

• Antihistamines relieve itching, sneezing and runny nose.
Common antihistamines include Benadryl, Chlor-Trimeton, Tavist,
Claritin

• Decongestants are often used in combination with
antihistamines and include Sudafed, Actifed

• Cromolyn sodium helps relieve hay fever symptoms by preventing
the release of histamine.

• Leukotriene modifier: A prescription tablet taken to block the
action of leukotrienes

• Nasal atropine helps relieve a severe runny nose

It is important to talk to your doctor before taking any
over-the-counter medications for hay fever especially if you
have a chronic health condition that requires you to take other
prescription drugs. You will need to know whether or not there
could be a drug interaction or other adverse effect.


If medications do not relieve your hay fever symptoms, your
doctor may recommend allergy shot therapy. The goal of
immunotherapy or desensitization therapy is to desensitize you
to specific allergens, reduce your signs and symptoms, and
decrease your need for medications. Allergy shots are usually
given over a period of three to five years to reach this goal.

Heavy drinkers for many years can experience permanent brain

Forms Of Treatment For Arthritis



Trying to figure out the best means to relief arthritis pain can
be frustrating. You can make your arthritis pain worse with
activities stressful to your joints and that require repetitive
motion when you already have severe joint damage. If you have
only mild joint damage, gentle exercise that includes stretching
and muscle strengthening can improve arthritis pain.

Rheumatoid Arthritis

Arthritis varies from one person to another. You will need to
work with your doctor to determine the right solution and
exercise program for your specific situation.

Activities such as jogging and playing tennis that put sudden
pressure on involved joints are likely to make the symptoms of
arthritis worse. Swelling and inflammation may increase.
Exercises that strengthen your muscles protect your joints and
reduce stress and joint damage should be of help to arthritis
symptoms. Strengthening the muscles on the front and back of
your thigh, which are the quadriceps and hamstrings; helps
protect your knee and hip joints.

The key is to do what you can to stay physically active while
taking into consideration the condition of your joints. You may
be able to walk a good 30 to 60 minutes at a comfortable pace,
but you probably will have to give up high-impact activities
like running that puts a lot of stress on many different joints.
Swimming or other water exercise may be a better choice for
keeping active until you get toned if your joints are too
painful for walking.

People with rheumatoid arthritis often have stiffness after
rest. Movement tends to diminish some of the symptoms. If you
experience excessive stiffness after sitting for a long time you
can try adjusting your sitting position often to help prevent or
lessen some of this.

You should call your doctor if new pain or persistent pain
lasting more than several days occurs. Arthritis symptoms are
often more effectively controlled if caught early. If your
symptoms are most likely due to overdoing it and they disappear
in a few days you probably do not need to call your doctor.

Medications for arthritis are relatively safe and well tolerated
by most people. Keep in mind there is no medication that is
completely free of possible side effects. If you experience
regular arthritis symptoms you need professional advice from
your doctor about which medications to take and how much.

Medications available include:

•Tylenol
•Aspirin
•Ibuprofen
•Advil
•Motrin
•Aleve
•Naproxen
•Other anti-inflammatory drugs

Most of the above drugs can be purchased over-the-counter and
many of them do bring relief of minor and occasional arthritis
pain.

A weekend tennis player, gardeners and other people with mild
osteoarthritis can overcome some of the stiffness by taking an
over-the-counter NSAID tablet for one or two days.

If symptoms are prolonged and severe, joint involvement may be
more advanced. Your doctor is the best source to decide the
dosage that is right for you.

Alternative treatment for arthritis pain:

Heat, massage and stretching help relieve arthritis symptoms for
many people. The best of these treatments should have your
doctor or physical therapist’s approval. These treatments have
also been standard treatments and not so much “alternative”
treatment for arthritis pain.


It may take years to find an effective and safe medication for
your particular arthritis pain. If you try a shortcut standard
practice it may lead to harmful effects and wasted money, time
and effort.

Botox Injections:

It is unclear at this time whether or not Botox injections
improve pain and function in people with arthritis. Much more
research is needed to evaluate the findings of a very small
study that suggested injections with Botulinum toxin type A
(Botox) may improve pain and function in people with arthritis.

The study reported improvements in pain and function in all
participants, however the duration of these benefits varied from
three to 12 months. The results were promising and there were no
adverse reactions reported. There will need to be much larger,
controlled and randomized trials to determine the safety and
possible benefit of Botox injections for arthritis.

Antibiotics for the treatment of rheumatoid arthritis:

Researchers continue to study the possibility that rheumatoid
arthritis may be brought on by some form of infection. If this
is true, it may be possible to prevent or stop the progression
of the disease with antibiotics. Studies of the usefulness of
antibiotic therapy have produced mixed results.

Effects of long-term use of antibiotics are unknown. Antibiotics
have many potential side effects, some of which are very
serious. More research is needed to determine if antibiotics can
be useful in the treatment of rheumatoid arthritis.
