Low Back Pain Causes and Treatments

Lumbar spine pain is a common complaint. Lower back pain is a major cause for lost productivity and activity restriction. Reasons for lower back pain include, but not limited to:


  • pregnancy
  • obesity
  • poor physical fitness
  • high levels of stress
  • anxiety
  • depression
  • children can suffer from backpack overload
  • strains
  • sprains
  • disc problems
  • inflammation, injury or infection in the abdominal organs including the bowels, reproductive system and urinary tract
Most people are uncertain about the cause of their back pain. The first place to start is with  your primary care physician. Doctors can only diagnose from what you tell them in addition to clinical examinations and tests. Keep a diary of your pain, when it occurs, where it occurs, how long it last, and describe it in detail. Take the diary with you when you visit your doctor. It is more difficult to explain symptoms after they occur then while they are occurring. Determining the exact cause of your back pain will determine course of treatment.

Clinical examination will include:
  • your back
  • pelvic area and legs
  • nerves and blood supply
If necessary your doctor may then go on to further investigations that include:
  • blood tests to look for general markers of illness and inflammation
  • specific tests such as HLA-B27 gene for ankylosing spondylitis and rehumatoid factor for rehumatoid arthritis
  • x-rays
  • CT scan
  • MRI
  • Myelography
  • bone scans
  • electromyography and nerve conduction tests
Other specialists that may be called in are:
  • neurologist
  • neurosurgeon
  • orthopedist
  • rheumatologist
  • physical therapist
When to call your doctor:
  • sudden onset of severe lumbar pain
  • intense back pain that lasts several days and prevents you from doing everyday tasks
  • lumbar pain with fever and/or sweating
  • sudden or increasing pain in legs
  • bowel or bladder incontinence
  • impotence
  • numbness in anal area or groin
Lumbar Pain Treatment may include:
  • home remedies (applying heat and/or cold to the painful area to help relax tense muscles)
  • over the counter pain relief with non-steroidal anti-inflammatory drugs such as ibuprofen
  • rest, but not more than a day or so
  • stay clear of high-impact or twisting exercises when you have acute lumbar pain such as running, basketball, rowing and even golf
  • strengthen your core, but not in acute stage of back pain, a strong core can help prevent back pain as weak muscles in the lower abdomen and pelvis is a common cause of strains, sprains and disc problems
  • physical therapy can help strengthen the core, improve mobility and flexibility and promote better posture
  • massage
  • acupuncture
  • spinal manipulation
You might be able to get recommendations from local friends for a good therapist. If you don't see considerable improvement within six sessions of any approach, then it is unlikely the approach is helping.

Your doctor may recommend a variety of other treatments dependent upon your diagnosis. These include:
  • biofeedback
  • nerve block therapies
  • epidural steroid injections
  • transcutaneous electrical nerve stimulation
  • surgery when other treatments fail
There are a variety of surgical procedures that include:
  • vertebroplasty
  • spinal laminextomy 
  • discectomy
Treatment always depends upon professional diagnosis.

Your best course of action is to hopefully prevent back pain. Walking is probably one of the best exercises to prevent most back pain problems as it helps to strengthen your core muscles. Other exercises that strengthen the core muscles done regularly can make a huge difference in overall posture and preventing common back pain ailments.
